Please use this space to inform us if your child has any health conditions that may affect him/her whilst at school or which we should be aware of:

  • Dietary

  • Health and Safety: Medication to Students

If your child requires regular medication, it should, wherever possible, be administered at home.  However, if your child needs this medication during the school day, it must be handed in to the school office as soon as your child arrives in school, together with written details of the dose, frequency and any other relevant information.   All medication, including over the counter pain-killers, must be in the original packaging and include the safety information insert.  Medicine containing aspirin is not suitable for under 16s and the school cannot accept this medication unless prescribed by a doctor.

If your child suffers from asthma and needs to carry an inhaler, please tell us in the Medical section above.

If your child has an Epipen or diabetic syringe, these should be brought into school in a clearly named lidded plastic box for secure storage in the school office.

Please phone the Admin Office if you have any queries.

  • Permission for visits / trips during the school day

From time to time your child will be given the opportunity to participate in local excursions for routine sporting fixtures, environmental studies, library and primary school visits etc. which are organised by the school.  Please sign below to give your consent:

  • Image Consent

Celebrating the successes of our school community is important and as part of this work we regularly take pictures and videos of school events.  These may then be used in various communications, for example on our school website, in our school newsletters, on our official school Facebook page and in any press releases authorised by the school.

To comply with the current General Data Protection Regulation (GDPR) we require your permission before we can photograph or make any recordings of your child. You or your child are able to withdraw this consent at any time.

  • Use of Biometric Information

To reduce queuing times at our food outlets and remove the need for students to carry individual cards, we use biometric information for our catering service.

On their first day at Monk’s Walk School your child will have his/her finger or thumbprint scanned and an account for catering will be created and linked to the payments you make through the school’s online payment system.

If you do not provide consent, an alternative method of access will be available to your child in the form of a PIN number.

You or your child are able to withdraw this consent at any time and the biometric information held by the school will be destroyed. In line with GDPR the school will periodically ask for this consent again in order to maintain up-to-date records. When your child leaves the school the biometric information will be destroyed.
